Chronic Care Management

• Chronic Care Management is a service and benefit of Medicare to help providers manage patient’s chronic conditions more closely with the goal of keeping patients out of the hospital and having regular correspondence with their specialist for a more holistic approach to their care.

Counseling and Cognitive Integration Services

• CoCM is a program that is available for our patients at CFM.  It is supported and encouraged by Medicare in order to improve patient outcomes and quality of life. The program enhances “usual” primary care by adding two key services, counseling and cognitive integration, to the primary care team, particularly regarding patients whose conditions are not improving.

Allergy Shots/Drops

• We offer allergy testing and treatment including weekly injections.

Osteopathic Manipulation Treatment

• OMT, is a set of hands-on techniques used by osteopathic physicians (DOs) to diagnose, treat, and prevent illness or injury. Using OMT, a DO moves a patient’s muscles and joints using techniques that include stretching, gentle pressure and resistance.

Weight Management

• Our program encompasses the physical, emotional, and cognitive aspects of weight management. Our goal is to help the individual make sustainable lifestyle changes that improve long term health. These lifestyle changes include improving one’s relationship with food, increasing nutrition knowledge, and reducing stress. Making lifestyle changes has been shown to improve physical health, reduce the need for medication, and improve emotional well-being.
